A Health Summary is a simple step that helps us provide the right care for you.

At NuLeaf Clinics, our goal is to make your healthcare journey simple, supportive, and informed. One of the first things we ask new patients to do is submit a recent Health Summary from their regular GP, referring doctor, or any relevant healthcare professional, such as a specialist, psychologist, pain specialist, holistic practitioner, or physiotherapist.

You might be wondering why this is needed before your consultation? Let’s walk through it.

It gives us the full picture

Your Health Summary includes important information about your medical history, any current or past conditions, and your prescribed medications. This helps our qualified practitioners understand where you’re coming from so they can make safe, informed decisions about your care. Think of it like setting the foundation for your appointment. The more we know upfront, the more personalised and relevant your consultation will be.

It supports safe and appropriate care

At NuLeaf, every decision we make is grounded in clinical judgment and your individual needs. Your Health Summary helps our team determine whether a health care plan is suitable or if other steps are recommended. Submitting it before your consultation means we’re better prepared to help you from the moment we connect.

What if I don’t have one?

If you’re unsure how to access your Health Summary, don’t worry, it’s easy. You can request a copy from your GP clinic, often by phone or email. Some clinics can also send it directly to us with your permission. And if you don’t have a regular GP, let us know when booking. We’ll guide you on what you need to provide instead.

A small step that makes a big difference

Providing your Health Summary is just one part of how we keep your care personalised and safe. It’s a simple step, but one that ensures your consultation is meaningful, relevant, and guided by your health history. Patients only need to submit their Health Summary once. However, if there are any significant changes or new health concerns, an updated summary may be required at the discretion of your practitioner.

At NuLeaf Clinics, we believe healthcare should be simple, supportive, and accessible, no matter where you are. Whether you’re starting your journey for general health, alternative health support or weight loss management, we’ve designed a process that keeps you informed and cared for every step of the way.

Here’s what to expect when you book a telehealth consultation with NuLeaf Clinics.

Step 1: Book your consultation

Your healthcare journey with NuLeaf begins by booking an appointment through our secure online platform. You can choose a time that suits you and select a consultation for the service most relevant to your needs—whether that’s general health, alternative health support, or weight-related concerns.

Step 2: Share your health information

To provide safe and appropriate care, we ask all new patients to submit a recent Health Summary from their regular GP, or a referral if available, and send it to hello@nuleafclinics.com.au. This information helps our practitioners make informed clinical decisions during your consultation and ensures continuity of care.

Step 3: Attend your telehealth appointment

All NuLeaf consultations are conducted via secured telehealth. Most appointments are by phone, with the practitioner calling from a private number. If preferred, a video consultation can be arranged upon request. You’ll speak with an Australian-registered healthcare practitioner who will take the time to understand your medical history, current symptoms, and health goals.

During your appointment, the practitioner will:

  • Review your submitted health summary and any past health care
  • Ask about your symptoms and lifestyle
  • Determine if a health care plan is clinically appropriate for your needs

If required, the practitioner may discuss suitable options, which could include lifestyle changes, supportive therapies, or if clinically appropriate, prescription health care.

Please note: Not all consultations result in a prescription.

Step 4: Follow-up and ongoing support

Our patient care doesn’t end after one consultation. Depending on your health care plan, we recommend a follow-up appointment around four to six weeks after your initial consultation. This allows your practitioner to check in on your progress, answer questions, and adjust your plan if needed.

If you’ve been prescribed medication, follow-up appointments are also an opportunity to review its effectiveness and manage any ongoing repeats, if appropriate.
